About Scheme
Ayushman Bharat - Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) is a flagship health assurance scheme launched on 23rd September 2018 in Ranchi, Jharkhand by the Hon’ble Prime Minister of India. It aims to provide a health cover of ₹5 lakh per family per year for secondary and tertiary care hospitalization to over 10.74 crore poor and vulnerable families (approximately 50 crore beneficiaries).
Details
The scheme has two components: (1) Health and Wellness Centres (HWCs) for comprehensive primary care, and (2) Pradhan Mantri Jan Arogya Yojana (PM-JAY) for hospital care. PM-JAY is the world's largest health assurance scheme, fully funded by the Government of India and implemented jointly by Centre and States based on SECC 2011 data.

Eligibility
Rural: Families falling under any of six deprivation criteria in SECC 2011 and those automatically included (destitute, manual scavengers, primitive tribal groups, bonded labourers). Urban: Families working in 11 occupational categories such as ragpickers, domestic workers, construction workers, street vendors, drivers, electricians, tailors, etc.
Benefits
Provides cashless cover up to ₹5,00,000 per family per year for listed secondary and tertiary conditions. Includes expenses for medical examination, hospitalization, surgery, medicines, implants, diagnostics, accommodation, food, complications, and 15-day post-hospitalization care. No cap on family size or age; all pre-existing diseases covered from day one.
Application Process
Offline: Beneficiaries visit hospitals or Common Service Centres (CSCs). Step 1: Search by Name/RC Number/Mobile/RSBY URN. Step 2: Search in BIS Application. Step 3: Individual Identification using Aadhaar or any government ID. Step 4: Family Identification using ration card. Step 5: Approval/Rejection by State Health Agency. Step 6: Upon approval, e-card is issued for cashless treatment. Beneficiaries can also apply via the UMANG app.

1. Aadhaar Card or PAN Card (Identity Proof). 2. Proof of Address. 3. Contact Details (Mobile, Email). 4. Caste Certificate. 5. Income Certificate. 6. Family Status Document (Joint/Nuclear). 7. Ration Card or Family ID.
